NEW PATIENTS
APPOINTMENTS
1ST VISIT & INTAKE PAPERWORK
As a new patient we will have you fill out some intake paperwork at your 1st first visit. We ask that you arrive 15 mins early to allow time to complete paperwork.
INSURANCE
Our office does not accept assignment from insurance companies. We will, on request, issue you a receipt that you may turn into your insurance company if you wish to make a claim for reimbursement (to be made directly to you). We do not submit paperwork directly to insurance companies.
PAYMENT METHODS:
We accept cash, checks, HSA cards and major credit cards.
Appointment Types
NEW PATIENT
Paid per visit
For 1st Time or New Patient Visit
A new patient visit is required by law if it has been more than 2 years since a patient’s last visit to our practice.
ESTABLISHED PATIENT
Follow up visit for established patients
Double visits are available on a limited basis when distance from our office or other special scheduling considerations may apply.
PREPAID PACKAGES
3 visits
6 visits
Prepaid packages cover visits, but not durable goods, such as supplements.
An initial visit counts as two visits on a package.
Packages may be used by an individual, or may be shared among family members, friends or business associates. We reserve the right to discontinue offering packages.
According to Texas Chiropractic Law, packages must have start and end dates and their duration must be based on a reasonable treatment plan. Texas Chiropractic Law, and Head 2 Foot Policies are both subject to change.
Policies
CANCELLATION & SCHEDULING POLICIES:
We require 48 hours notice BY PHONE ONLY to cancel an appointment without charge.
Missed appointments (not canceled or rescheduled at least 48 hours in advance (BY PHONE ONLY. NOT BY EMAIL!) will incur a missed appointment charge.
Our Front Desk staff makes sure that you are notified and reminded of your appointments by email and text message. It is each patient's individual responsibility to keep track of the correct date and time of all your appointments and to arrange to arrive on time or slightly early for any appointments you have scheduled.
There may be someone scheduled after you, and your appointment may be shortened if you arrive late.
If you are too late to be seen, we will have to reschedule you, and charges for the missed appointment will still apply.
We ask the patient to be mindful of arrival time and be ready to enter the treatment room when called by Front Desk Assistant.
